VOA is a faith based, non-profit organization dedicated to helping individuals, families and communities reach their full spiritual, emotional and physical potential and well-being through a vast array of human services that target the elderly, at risk youth, those striving to overcome substance abuse, those transiting from correctional centers, veterans, homelessness and more.

Essential Job Functions Include

  • Work with low-income Veteran Families and providing supportive services related to homelessness prevention and re-housing.
  • Conduct outreach and awareness through partnerships and collaboration with other community service providers and human service agencies.
  • Develop a trusting and helping relationship with Veterans and their families.
  • Inform Veterans of all available resources.
  • Conduct a formal needs assessment and social history on each Veteran (and family) who presents for services, working in conjunction with staff from VAMC, DVOP and LVER when appropriate.
  • Regularly coordinates intakes of new participants.
  • Develop and follow up on housing and employment plans with each Veteran.
  • Remain knowledgeable about specific resources available for Veterans, making referrals and performing follow up as appropriate.
  • Work with the VA to determine eligibility for services and other benefits, to secure DD 214 forms and to develop linkages with other service providers.
  • Assure access to and/or provide necessary training and support services for Veterans and families.
  • Work with Veteran and families along with other service providers to make sure the basic needs of food, shelter and clothing are met.
  • Document all services provided and collect/track data relating to performance goals, along with quality assurance of services offered.
  • Communicate with data coordinator on data being entered into HMIS and other databases.
  • Provide housing placement assistance for SSVF Clients focusing on household's needs and goals.
  • Perform SSVF Housing Inspections prior to move-in, assist in negotiation of rents.
  • Provide assistance and expertise to landlords or SSVF staff for conflict resolution and problem solving.

 Other Job Functions Include:

  • Outreach and community partnering with other civic and Veteran's groups.
  • Developing a referral network for Veterans and families throughout the various communities where services are provided.
  • Assist Veterans and families in the utilization of transportation and perform transportation duties as necessary.
  • Must be able to travel as required.
  • Attends all staff meetings and trainings as requested.
  • Other duties as assigned.

 Physical Demands:

  • Ability to move or transfer items throughout the facility generally weighing 0-50 pounds.
  • May be required to sit at a computer station for extended periods of time to complete essential job functions.

 Special Knowledge, Skills, and Abilities:

  • Ability to work with potentially difficult people in a confrontational and/or emergency situation.
  • Effective oral and written communication skills.
  • Ability to work independently and as a team member.
  • Computer skills including Microsoft Office.

 Minimum Qualifications:

  • A Bachelor's degree in human services, behavioral science or related field with one year of case management experience


  • An Associate's degree with a minimum of two years case management or related experience


  • High School diploma or equivalent and minimum four years case management experience or related field experience.
  • Must have a valid driver's license and complete a Federal Criminal Background Check with approved status.
